๐Ÿ’Š Clinical snapshot: Warfarin

A pharmacist's summary of the drug in this name. Facts marked [C] link to FDA labeling on DailyMed, MedlinePlus or other NIH/NLM and FDA pages.

What it is. Warfarin is an oral vitamin K antagonist anticoagulant ("blood thinner") with a narrow therapeutic range.[C1][C2]

FDA-approved uses. Prevention and treatment of venous thrombosis and pulmonary embolism; preventing clots from atrial fibrillation and mechanical heart valves; reducing death, recurrent heart attack and stroke after a heart attack.[C1][C2]

How it works. It inhibits vitamin K epoxide reductase, depleting active vitamin K and reducing synthesis of clotting factors II, VII, IX and X and proteins C and S.[C2]

Forms and strengths. Tablets 1, 2, 2.5, 3, 4, 5, 6, 7.5 and 10 mg, each a different color.[C2]

Key warnings and side effects. Boxed warning: major or fatal bleeding. Regular INR monitoring is required, and many drugs, supplements, illnesses and changes in vitamin K intake alter its effect. Contraindicated in pregnancy (except some with mechanical valves), active bleeding, recent CNS or eye surgery, and situations where bleeding risk outweighs benefit. Skin necrosis and purple toe syndrome are rare complications.[C1][C2]

โš•๏ธ Not medical advice. This summary is general information, not a substitute for your own clinician. Labeling notes changes in vitamin K intake affect INR, which is monitored regularly. Ask your pharmacist before starting any new drug, antibiotic or supplement. Talk to your doctor or pharmacist before starting, stopping or changing any medicine.
